The Medical Records/ Health Information Department is responsible for the maintenance and safeguarding of every patient's medical record for each of their visits to the hospital. When a patient requests a copy of his/her record, the H.I.M. departments first verifies the patient's identity and once verified will produce a copy of the requested record.

To Obtain Your Medical Record:

To ensure the privacy of patients and their families and to comply with federal regulations, it is necessary to have patients sign a release of information form to obtain medical records.

In order to obtain a copy of your record, contact Medical Records Monday-Thursday 8 a.m.- 5 p.m. and Friday 8 a.m.- 2:30p.m. at 606-598-5104 ext. 3203. Medical Records will need your name, date of birth and photo ID. 

The charges for medical record copies are based on fees allowed by Kentucky State Law. The first copy of your medical records is free. Any subsequent copies of your medical records cost 99 cents per page.

How do I get a copy of a birth certificate?

Birth certificates must be obtained from the Commonwealth of Kentucky. You can order copies of birth certificates at www.vitalcheck.com. For more information contact Kentucky Vital Records at 502-564-4212.
