Employee vision discount available through Dec. 31, 2006
Employees at Memorial Hospital can take advantage of the vision discount available through Humana. This benefit is available until the policy year ends on December 31, 2006. In order to receive the discount you must go to an EyeMed provider.
To receive your discount, present the discount card at your provider's office or location. There are no claims to file and no waiting for reimbursement. The discount will be applied directly to your purchase. If you forget your discount card, some providers may accept your Humana member ID card in place of the discount card.
